We can close the health equity gap by reducing health disparities by increasing physical accessibility to care, improving availability of appropriate care and building more inclusive public health programs and communities. #NPHW2022

Here at GCHD we offer services for many different people and their specific needs. Two programs we will be introducing today are Care Connect and (CYSHCN) Children & Youth with Special Health Care needs. 

The Care Connect program helps those who tested positive for Covid-19 or if they were exposed to someone that tested positive. This program focuses on eliminating barriers families face while isolating and quarantining safely at home. By providing families with necessities such as fresh food order and financial assistance we are reducing the spread of COVID-19 as well as helping families recover. Care Connect services are available to Grant County residents at no cost if you qualify. To find out if you qualify, call 1-800-525-0127, then press #. 

What is the Children & Youth with Special Healthcare Needs Program? 

The mission of the WA State CYSHCN Program is to promote integrated systems of care that assure the population of children with special health care needs the opportunity to achieve the healthiest life possible and develop to their fullest potential. 

At the State level, the CYSHCN program collaborates with families, policy makers, health care providers agencies and other public-private leaders to address health system issues that impact CYSHCN families. At the Local Level CYSHCN Coordinators help families access needed services for their child through case management.
